Targeted Case Management

Case Management services are those that assist the individual in gaining access to medical, social, educational, and other needed services.  A Targeted Case Manager's (TCM) role includes any or all of the following services: 
Assessment of an eligible individual to determine service needs
Referral and related activities

Monitoring and follow-up activities to ensure care plan is implemented

Transition and portability, including the planning of and arranging for services
Development of a Person-Centered Support Plan

A PCSP is a written plan developed by the individual consumer and his/her support network, that involves a well thought out strategy of support designed to help the individual live their life in a manner they prefer.  A PCSP includes topics such as: Type of setting she/he wants to live in, Who the individual wants to live with, Work or other valued activities, and Social, leisure, religious or other activities
